Tardiness, Missed Appointments
The time you have scheduled for your appointment is your financial responsibility, and you are required to pay for your whole appointment, whether you use the time or not. I do not "overbook" and cannot "squeeze" another patient into your scheduled time if you are late or do not show up for your appointment.

If you are late for your appointment time, I will see you for the remaining time of your appointment. For example, if your appointment is a 2:00 pm medication check-up but you do not show up until 2:15 pm we may only have 10 to 15 minutes left of your time. If you are late, I will make sure sure we cover the most pertinent information during your appointment time, but we may not have time to cover all the problems you may wish to discuss.

If you do not show up for your appointment during your scheduled time I will be happy to schedule a new appointment for you. If you fail to show up for your appointment or do not cancel your appointment with 24 hour notice, you will be responsible for the full fee of the appointment unless I am able to fill your appointment time. I do not have a secretary who can call you to remind you of your appointment time. It is your responsibility to remember your appointment.

Financial Responsibility
Payment is expected and due at each visit. Unfortunately, financial noncompliance may interrupt your care. I will attempt to work with you regarding financial matters if needed.

Insurance Claims
Of course, you may file claims with your own insurance provider and I would be happy to complete my portion of the paperwork and return it to you. Please note that if your insurance company denies your claim it has no bearing on your financial responsibility to me. Also, my fees are set by me and are not affected by your insurance company's "reasonable and customary fees".

Medication Refills
Medication refills can be requested by calling (813) 250-0224 extension 5#. To ensure that you do not run out of medication, please give me a minimum of five business days to fill your prescriptions, as I am not in my office every day. I do check my messages periodically during each day, and I will try to respond to your request promptly.
